27 year old female security staff presented with bilateral ankle swelling and erythema nodosum lesion over shin since 3 months, CXR was suggestive of enlarged mediastinal lymph nodes

CT chest showed enlarged pre tracheal, mediastinal lymph nodes

Provisional diagnosis of Acute sarcoidosis Lofgren syndrome was made and patient responded to treatment well
